About the role
Hospice Social WorkerThis role delivers varied social work services to Hospice patients and their families in Philadelphia. You will provide initial emotional, spiritual, psychosocial assessments, ongoing counseling, bereavement services and community education, outreach and referral. The Hospice social worker is an integral part of the Hospice IDG.The service area for this position will cover Philadelphia and Bucks County. Responsibilities include performing initial psychosocial, emotional, spiritual and bereavement assessments and assisting in the development and implementation of goal-directed IDG care plans. Conduct ongoing reassessments of patient/family needs and counseling as required. Provide short term crisis intervention and individual or family counseling when indicated. Participate as a member of the Bereavement Team as assigned. Observe, assess and bring to IDG conferences information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family. Assume the active role of advocate for the patient/family unit. Develop and maintain contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals. Document comprehensive psychosocial, emotional and spiritual assessment clearly and concisely in a timely manner. Record all patient/family visits, telephone contacts and referral actions in the patient record per policy. Provide ongoing counseling related to issues of death and dying to the patient and family as needed. Attend staff meetings, IDG and other meetings as assigned and appropriate. Participate in the orientation program as assigned. Adhere to all Hospice policies. Assume responsibility for own personal and professional development and maintenance of skills in social work. Exhibit Hospice philosophy in all job related roles. Other duties as assigned by Director/Manager of Patient Services. Qualifications include a Masters in Social Work, experience working in the Hospice field, a valid driver's license, and the ability to travel across the service territory.
