This is a full time opportunity rounding and providing leadership in skilled nursing facilities. Perform comprehensive patient assessments, including medical histories and physical exams. Develop, implement, and adjust individualized care plans based on patient needs. Educate patients and families on health management and preventive strategies. Maintain accurate, timely medical records in compliance with regulations.
