This is a field-based role focused on outreach, engagement, housing transitions, and ongoing support in the community.
What you’ll do: Conduct outreach and build trusting relationships with individuals in the community Provide short-term counseling, crisis intervention, and supportive services Complete psychosocial and risk assessments and develop individualized care plans Support participants before, during, and after transitioning into housing Connect individuals with healthcare, housing, and community resources Coordinate with hospitals, outreach organizations, social services, and other community partners Help participants navigate appointments and overcome barriers to care Support case managers and peer staff as part of a multidisciplinary team Document progress and contribute to team meetings and care planning
What we’re looking for: Master’s degree or higher in social work, mental health counseling, psychology, nursing, or a related field Appropriate professional licensure required Experience with individuals experiencing homelessness, mental health conditions, or substance use disorders is a plus Strong counseling, crisis management, de-escalation, and community outreach skills Person-centered, trauma-informed, and harm-reduction approach Strong communication, documentation, and collaboration skills Comfortable working in the field with flexible hours and occasional on-call coverage Why join?
