About the role

Signify Health, part of CVS Health, is seeking a Full Time Clinician (NP or PA) to provide in-home health evaluations across a defined coverage area. Travel up to 100% of the time with potential overnight stays and flexible to licensing requirements. You'll perform assessments, review medications and history, and use an iPad-based workflow without altering existing treatment plans unless directed by the health plan.
