Responsibilities: Conduct clinical assessments to identify patient needs and determine appropriateness of physical therapy. Develop, implement, and periodically revise a Plan of Care (POC) with progress notes and related documentation; collaborate with physicians and an interdisciplinary team. Plan and deliver therapy for neuromuscular, neurological, and orthopedic conditions based on medical referral and evaluative data. Instruct patients and families in safe transfer techniques and the use of equipment (walkers, wheelchairs, canes, braces, prosthetic devices); select and adapt equipment as needed for home environments. Provide patient and family education tailored to age, condition, culture, and learning readiness; promote infection control and universal precautions. Coordinate with and, when applicable, supervise other team members to ensure care aligns with patient needs and preferences.
