Home Health Registered Nurse in Franklin, TNAn excellent opportunity awaits for a skilled and compassionate Registered Nurse with proven experience in home health care. This full-time, 13-week contract role focuses on delivering exceptional, patient-centered care directly in patients homes, supporting positive clinical outcomes and continuity of care.
Schedule: Monday Friday, 8:00 AM 5:00 PMAs a Home Health RN, you will manage a caseload of 2532 points per week (typically 35 patient visits per day) within a designated territory, using the Home Care Home Base (HCHB) electronic documentation system and the OASIS assessment tool. Mileage reimbursement supports your daily travel to each patients home.
Key Responsibilities:
  • Perform comprehensive OASIS assessments and develop individualized plans of care
  • Deliver skilled nursing services such as medication administration, wound care, IV therapy, and chronic disease management
  • Monitor patient progress, recognize changes in condition, and quickly update the care team and physicians
  • Educate patients and their families about health conditions, care plans, medications, and preventive care
  • Maintain accurate and timely documentation in HCHBCollaborate with therapists, social workers, and home health aides to ensure coordinated, multidisciplinary care
  • Attend case conferences and participate in professional development activities
  • Adhere to all regulatory and agency standards for patient care and documentation
Required Qualifications:
  • Active, unencumbered RN license for the state of practice
  • Minimum 1 year recent home health nursing experience, preferably field-based
  • Proficient in using HCHB and OASISStrong assessment, clinical judgment, and organizational skills
  • Reliable transportation, valid drivers license, and insurance
