About the role
- Provide one-on-one social service assistance, information, referrals, and follow-up to participants.
- Assist older adults, residents, families, and community members with accessing benefits, healthcare, food resources, transportation, housing resources, and other community services.
- Conduct participant outreach, wellness calls, appointment reminders, and follow-up.
- Help participants complete applications, forms, and service referrals.
- Support case management and service coordination activities under the direction of program leadership.
- Maintain accurate and timely participant records, service notes, referrals, and other required documentation.
- Assist with senior programming, community activities, workshops, outreach events, and participant engagement.
- Provide temporary coverage at BHNC program locations when regular staff are unavailable.
- Work collaboratively with program staff, property management partners, community organizations, and other service providers.
- Identify participant concerns or urgent needs and appropriately escalate them to program leadership.

- Are comfortable moving between programs, locations, and assignments.
- Can step into an unfamiliar situation, assess what is needed, and get to work while following established procedures.
- Have strong communication, organization, documentation, and follow-through skills.
- Can maintain professional boundaries and confidentiality.
- Are comfortable using computers and electronic systems for documentation and communication.
- Are dependable and able to work both independently and as part of a team.
- Experience working with older adults, low-income communities, affordable housing residents, immigrants, people with disabilities, or individuals navigating complex service systems is highly valued.
- Bilingual or multilingual skills are strongly desired. What Makes This Position Different This is not a traditional position assigned to only one program. The Social Services Floater is part of BHNC's strategy to maintain strong, consistent services across our programs.
- Your primary assignment may change based on organizational and participant needs. Some weeks you may support senior services; at other times, you may assist the Service Connection team or provide short-term coverage where additional social service capacity is needed.
