About the role
Avail Health Medical Practice is seeking a full-time Mobile Nurse Practitioner to support I-SNP members in northern West Virginia. This hybrid, field-based role serves skilled nursing facilities and requires commuting to assigned sites with a Monday–Friday schedule and HIPAA-compliant telework.
You will perform HRAs, develop care plans, coordinate with IDT, manage transitions of care, and maintain compliant documentation.
