Home Based Primary Care Case Management Registered NurseThe Home Based Primary Care (HBPC) Case Management Registered Nurse provides evidence-based nursing care and case management to Veterans in their homes within a 60-mile radius of the Ottumwa VA Outpatient Clinic. Nurse performs assessment, triage, education, resource coordination, and care planning for Veterans with complex needs, including primary, geriatric, preventive, and palliative care.
