About the role
- Conduct pulmonary assessments for patients in post-acute care settings
- Evaluate, diagnose, and manage pulmonary conditions including COPD, asthma, pneumonia, and respiratory failure
- Develop and implement individualized treatment plans
- Collaborate with interdisciplinary teams
- Educate patients, families, and facility staff on respiratory care and treatment plans
- Monitor patient progress and adjust care plans as needed
- Maintain accurate and timely EMR documentation
- Support initiatives focused on reducing hospitalizations and Return-to-Hospital (RTH) events
- Preferred a minimum of 6 months of experience in post-acute care setting
- Preferred experience in Pulmonary: training provided
- Active and unrestricted state Nurse Practitioner license or Active PA license and NCCPA certification required
