Nurse Practitioner / Physician Assistant – Skilled Nursing & Long-Term Care
Location: Eugene, OR and Billings,
MTPosition Overview:
We are seeking a compassionate and clinically driven Nurse Practitioner (NP) or Physician Assistant (PA) to provide comprehensive medical care to residents in skilled nursing and long-term care facilities throughout the Eugene area. The provider will manage acute and chronic conditions, perform routine and post-acute assessments, and collaborate closely with physicians and interdisciplinary care teams to improve patient outcomes and quality of life.
Key Responsibilities:
Conduct admission evaluations, comprehensive assessments, physical examinations, and follow-up visits. Diagnose and manage acute and chronic medical conditions. Develop individualized treatment plans and prescribe medications as appropriate. Order, review, and interpret laboratory tests, diagnostic studies, and imaging. Provide post-hospital follow-up and transitional care management. Monitor changes in cognitive, behavioral, functional, and medical status. Educate residents, families, and caregivers on diagnoses, treatment plans, and preventative care. Collaborate with physicians, nursing staff, therapists, social workers, and other healthcare professionals. Support initiatives aimed at reducing hospital readmissions and unnecessary emergency department visits. Maintain accurate and timely EMR documentation in compliance with CMS, state, and facility regulations.
Requirements:
Graduate of an accredited Nurse Practitioner or Physician Assistant program. Active Oregon NP/PA license or ability to obtain prior to start. National board certification or board eligibility. DEA registration and prescriptive authority, or ability to obtain. Current NPI number. Eligibility to participate in Medicare and Medicaid programs. Current BLS certification. Authorization to work in the United States.
Preferred Qualifications:
Experience in skilled nursing, long-term care, geriatrics, family medicine, internal medicine, or post-acute care. Experience managing medically complex and chronic-care patients. Familiarity with CMS regulations and documentation requirements.
