Senior Team Counselor: Resource Focus - ACCS ACCS Integrated Teams provide clinical interventions and peer and family support to facilitate engagement, support functioning and maximize symptom stabilization and self-management of individuals and to assist individuals in developing skills, establishing natural supports and resources to live successfully in the community. The Senior Integrated Team Counselor will work as a member of a Persons' Team having primary responsibility in delivering interventions identified in the treatment plan with clinical guidance from the Integrated Team clinical staff. This position will have a specialty role in at least one area of expertise which may include: building new relationships with community resources, research and connection with various benefits and resources, employment and education, or representative payee. The Senior Team Counselor will also be responsible for: Provide input to person-centered assessment and service planning, and advocacy, coordination, and linkage with other services and the community. Model and teach skills for development of positive relationships, and negotiation of conflict. Assist persons served in understanding, managing, and dispensing medications consistent with MAP and agency policies and procedures. Maintain regular daily, monthly, and annual clinical records in keeping with DMH, agency, and division policies and procedures, and within required timelines.

Schedule: Full Time, 40 hours
