About the role
Job Description:
As part of the Saint Luke's Physician Group (SLPG) Population Health Division, the Population Health Outreach Specialist collaborates with internal and external stakeholders to optimize health management for our attributed patient population. With a strong focus on primary care, this role plays an integral part in maximizing SLPG performance within value-based payer contracts and supporting high-quality patient outcomes. The ideal candidate understands the goals of population health management and value-based care and is passionate about improving healthcare delivery through proactive patient engagement and collaboration. This position also supports the Care Coordination Program across all SLPG Primary Care providers and clinics.
Work Schedule: Monday through Friday, daytime business hours with 8-hour shifts.
Work Location:
This position is based at Blue Springs Primary Care. Hybrid Work Option: Employees may be eligible to work remotely up to 3 days per week after successfully completing their first 90 days of employment, provided productivity and performance expectations are being met.
Job Requirements
Applicable Experience:
