About the role

The Health Plan (THP) seeks a Director of Case Management to lead and manage all aspects of clinical program management, providing oversight and assessment to ensure the delivery of quality, member-driven care across the care continuum. The director ensures compliance with regulatory and accreditation standards while coordinating with the VP of Clinical Services, the Director of Utilization Management, and other medical leadership to advance population health initiatives.
