About the role
Yale NewHaven Health is seeking a Manager, Community Health to lead regional community health and health equity initiatives. You will assess needs, build partnerships with local organizations, and oversee CHNA/CHIP efforts while guiding system-wide health equity projects.
The role requires a Master’s in a relevant field and at least 5 years’ experience in community health, engagement, and program leadership.
