While DAH primarily serves residents living in communities managed by the Discovery Senior Living family of companies, its services extend beyond these communities. Responsibilities:
- Conduct initial evaluations to assess patients' needs and develop individualized treatment plans
- Develop and implement appropriate treatment plans based on patient needs and goals
- Monitor patient progress and adjust treatment plans as necessary
- Educate patients and caregivers on proper techniques to improve mobility, reduce pain, and prevent injuries
- Maintain accurate and timely documentation of patient progress
- Collaborate with other healthcare professionals to ensure patients receive comprehensive care
- Provide home health and/or outpatient services as needed, including supervisory visits, to ensure optimal outcomes
Qualifications:
- A degree in Physical Therapy from an accredited program
- Must be licensed or registered by the applicable state without restrictions
- One year of home health experience required
- Must have knowledge of Medicare and Medicaid guidelines
- Must have a valid driver's license and reliable transportation.
- Must have a current CPR certification
- Excellent verbal and written communication skills
- Basic computer skill proficiency. Ability to use Microsoft Windows
