About the role

Habitat Health is seeking a Home Care Coordinator to ensure personal and clinical homecare needs are delivered, helping participants thrive in their homes and communities. You will assess needs, develop care plans, and coordinate home care services in collaboration with the IDT to provide safe, person-centered care in the home setting. Role requires travel and a valid driver’s license; 80% of responsibilities occur in the community with required documentation in EHRs and adherence to policy.
