Responsibilities:
  • Conduct comprehensive physical therapy evaluations and develop individualized treatment plans for residents within the facility
  • Implement evidence-based therapy interventions to improve mobility, strength, balance, gait, and functional independence
  • Reassess patient progress regularly, adjust goals, and document outcomes in accordance with facility policies and regulatory requirements
  • Maintain accurate, timely documentation and collaborate with the interdisciplinary care team to coordinate resident care
  • Educate residents and families on home exercise programs, safety strategies, and fall prevention
Requirements:
  • Ability to provide physical therapy services in a skilled nursing facility setting
  • Strong clinical assessment, treatment planning, and documentation skills
  • Effective communication and teamwork abilities to coordinate with nurses, physicians, and other therapists
  • Willingness to work on-site in Silver City, NM and adhere to the assigned schedule
