What We’re Looking For:
We are currently seeking licensed Physical Therapists (PTs) and Occupational Therapists (OTs) to provide home therapy services throughout all five boroughs of New York City. Therapists can choose cases based on their preferred geographic areas and availability.
Must Have:
- Valid New York State professional license as a Physical Therapist or Occupational Therapist
- Degree from an accredited Physical Therapy or Occupational Therapy program
- Ability to travel to patients' homes within your preferred borough(s)
- Ability to work independently with minimal supervision
- Strong organizational and communication skills
- Excellent interpersonal and patient-care skills
- Valid driver's license and reliable transportation, when applicable
- One year of home health experience is preferred but not required
Responsibilities:
Evaluate patients referred for home care services to determine the appropriateness of physical therapy services.
Establish a home health treatment plan with clear goals based on the initial assessment, and provide timely, effective treatment.
Collaborate with agency personnel to ensure the agency meets the patient's needs.
Explain the agency’s policies and services to patients and their families during the initial home care assessment.
Complete and submit documentation accurately and promptly to the agency office.
Maintain ongoing communication with agency personnel.
Ensure customer satisfaction through professional, empathetic care.
Pay: $75.00 - $85 per visit
