WESTON UNITED COMMUNITY RENEWAL, INC.JOB TITLE: Licensed Practical Nurse (LPN) Per Diem/TempDEPARTMENT: Residential/DHSREPORTS TO: Program CoordinatorFLSA STATUS: Non-ExemptESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING:• LPN must supervise medication and ensure that consumers are compliant withmedication regimen. Prepare medication setup for consumers.• Monitor medication log and consumer medication sheets.• Work closely with pharmacies and the psychiatrist to ensure consumers obtain prescriptionsthat are filled in a timely manner.• Must ensure that documentation is accurate and provide feedback and/or concerns topsychiatrist and other staff.• Complete health assessments for new clients and annual assessments for currentclients (e.g., blood pressure test) for each consumer and enter all information into theclient electronic health record system AWARDS.• Candidate must counsel clients about medication non-compliance and other healthrelated matters as needed.• Other duties reasonably relevant to the position may be assigned.
QUALIFICATIONS:
Must possess a Nursing Degree and an NYS-licensed Practical Nurse (LPN) license is required; atleast two years of mental health experience and experience with adult consumers with co-occurring disorders. Candidate must have good interpersonal skills; excellent written and verbalcommunication skills. Proficient in Microsoft Office applications, including Word, Excel, andknowledge of the OHS CARES system and AWARDS is a plus.
SCHEDULE: Flexible schedule; days may vary. Minimum of 24hrs/week.
