Responsibilities: Provide personalized care and support to clients in their homes, assisting with daily living activities such as bathing, dressing, grooming, and medication management Develop and implement individualized care plans with clients, families, and healthcare teams to ensure continuity of care Assist with meal preparation, light housekeeping, and laundry as needed Monitor and report changes in client condition, medication side effects, and other relevant health information Maintain accurate and detailed records of care provided, client progress, and any concerns or issues that arise Collaborate with other healthcare professionals, including physicians, registered nurses, and occupational therapists, to ensure seamless care coordination Demonstrate knowledge and understanding of state-specific programs and regulations governing home care services About the Role As a Caregiver / Home Health Aide, you will have the opportunity to make a real difference in the lives of our clients.
