About the role
Advantmed is seeking a dedicated Nurse Practitioner to join our full-time provider network. In-home wellness and risk adjustment assessments across Oregon will be performed to support accurate documentation and high-quality, value-based clinical care.
The role requires travel within the state, performing health evaluations, wellness visits, and care coordination with our clinical team. Oregon NP license and 3+ years’ primary care experience are preferred.
