As our Transplant Licensed Social Worker, you will provide psychosocial assessment, intervention, and advocacy for transplant candidates and recipients and their families and caregivers. Every day you will work compassionately, with cultural competence and tact, to facilitate access to and continuity of transplant care, in a role federally mandated by UNOS for Medicare-certified solid organ transplant programs. To be successful in this role, you will bring social work licensure and transplant psychosocial expertise. Performs pre-transplant psychosocial evaluation of potential transplant recipients. Performs pre-transplant interval psychosocial re-evaluation of at-risk transplant candidates while listed for deceased donor transplantation. Provides short-term adjustment counseling to pre-transplant recipient candidates, to assist with emotional, social, and/or economic stresses of illness and disability. Facilitates pre-transplant referral to community resources as indicated for recipients. Documents pre-transplant assessments and interventions according to transplant center policy. Reports pre-transplant psychosocial findings and/or concerns to multi-discipline transplant team. Contributes to coordinated, interdisciplinary plans of care for recipients.
