About the role
We are seeking experienced Registered Nurses (RNs) with strong assessment, care coordination, and member education skills to support a Disease Management program. Candidates with prior Case Management experience, telephonic care management experience, and Medicaid knowledge are highly preferred.
Required Qualifications:
• Active Pennsylvania RN License• Disease Management experience• Telephonic Case Management experience• Strong assessment, care coordination, and member education skills
Preferred Qualifications:
• CCM (Certified Case Manager)• Medicaid experience• Previous Case Management experience Additional Requirement:• Candidates must complete the Care Manager Assessment in Glider.
