About the role

Minimum Qualifications: Associate degree with two (2) years of experience or equivalent in healthcare.
Required: Valid driver's license
Preferred Qualifications: Bilingual highly desirable.
Job Summary: The Community Health Worker assists UTMB Health patients and their families by addressing health and social challenges and educating the community about the diverse programs and services UTMB provides.
Job Duties: The primary point of contact for patient and community outreach activities related to a specific program. Educates the community and patients about a program and available services Develops relationships with community organizations, local outreach programs, and other patient- or program-specific initiatives. Assists and educates patients in navigating the healthcare system (including scheduling and keeping appointments, securing transportation, communicating with providers, and accessing community resources. Serves as a resource for patients and families by providing access to health information and community resources and addressing barriers to care. Communicate with healthcare providers about patient-specific health status as needed to ensure continuity of care. Create and implement community-wide marketing campaigns, including the development of marketing materials. Manage content for program-specific websites and social media accounts. When required, must be able to travel during business hours for patient care and community outreach needs. Adheres to internal controls and reporting structure. Performs related duties as required. Knowledge/Skills/Abilities: Good written and verbal communication skills in both English and Spanish.
Salary Range: Commensurate with experience. Working Environment/Equipment: Standard hospital, clinical, office environment, patient homes, and community resource agencies.
