About the role
SSG-Project 180 turns lives around through innovative, wraparound services that keep people out of institutions and in their communities. The Interim Housing program provides intensive, wraparound behavioral health services at four transitional housing sites. The Case Manager (Interim) role is field-based and works with a multidisciplinary team to support recovery and community integration. Under supervision, you will engage clients, coordinate care, and ensure documentation aligns with DHS/ODR
