About the role

This clinician provides telehealth follow-up visits for older adults needing psychiatric medication management, with a focus on depression, anxiety, dementia-related concerns, cognitive impairment, and other medication-related issues. Care also includes cognitive screening, initial care coordination planning, and ongoing monitoring for medically complex geriatric patients across assisted living, long-term care, and community settings. The role is supported by nursing staff and supervisory psychiatric collaboration within a team-based, remote practice model.

Key Responsibilities:

Conduct telehealth follow-up visits for geriatric psychiatric medication management.
Monitor treatment response and adjust medications with attention to polypharmacy, fall risk, and medical comorbidities.
Perform cognitive screenings and assessments using tools such as MoCA, MMSE, or similar instruments.
Develop initial care coordination and behavioral support plans.
Collaborate with nursing staff providing ongoing monitoring and patient support.
Communicate with the supervising psychiatrist as needed.
Maintain timely and accurate documentation in the EMR.Required
Current Florida license and certification as a Physician Assistant or Psychiatric Mental Health Nurse Practitioner.
Experience providing care via telehealth platforms.
Strong clinical judgment related to medication safety in older adults.
Ability to work collaboratively in a team-based care model.

Preferred: Experience in geriatric psychiatry, long-term care, or dementia care. Experience with cognitive assessment tools and behavioral management strategies. Experience working with older adults or in geriatric mental health. Bilingual in English and Spanish. Established relationships within the local community.
