Harmony Chicago is seeking a Social Services Designee to join our skilled nursing and post-acute care community. You will help prepare residents for discharge, address social and emotional needs, and participate in care conferences to ensure thoughtful resident support. Minimum qualifications include a high school diploma and 2 years of Social Worker experience in healthcare; a Bachelor's in Social Work is preferred with 5 years of related experience.
