About the role
A Healthcare Facility in the Syracuse, NY area is seeking an experienced MDS Coordinator / MDS Assessor to join our clinical team. Must Have MDS 3.0 Experience
Qualifications:
- Active New York State RN License required
- Previous experience in a clinical care coordination or assessment role preferred
- Strong organizational and time management skills
- Solid computer and documentation skills
- Excellent communication and interpersonal abilities
- Ability to work collaboratively within a multidisciplinary team
Responsibilities:
- Complete accurate assessments, MDS documentation, and care plans in accordance with regulations and facility standards
- Initiate and update individualized care plans as assigned
- Create and distribute monthly care plan schedules in a timely manner
- Maintain and update clinical assessments and documentation as required
- Monitor and audit clinical records to ensure accuracy, compliance, and timeliness
- Communicate documentation concerns and compliance issues to nursing leadership
- Maintain strict confidentiality of patient and organizational information
