About the role

Centurion Health seeks a full-time Transitional Healthcare Liaison to develop and implement transitional healthcare plans for incarcerated individuals scheduled for release. Collaboration with the Indiana Department of Correction ensures pre-discharge planning is completed for re-entering society. Role requires two years in a correctional setting, knowledge of community programs, and CPR/AED certification. Travel within Indiana is required. Benefits included with full-time employment.
