Shift Hours: 9am-5pm

Hourly rate: $18 on W2 + $46 (Travel Reimbursement) = $64.00/hrs Travel rate: $675+ $1,725.01 (travel reimbursement) = $2400.00

Job Description: Screens and assesses patients in an accurate and timely manner based on levels of care; determines patients' nutritional needs and status and provides follow-up care. Interviews patients and/or their families to obtain history of food habits and preferences. Interprets pertinent laboratory, physical and social data in the medical record, as it applies to nutrition care. Provides nutrition education and records patient's diet and diet instructions and the patient's understanding of the diet in the education section.

Experience: Minimal one year of experience. Skillsets: Electronic medical charting systems – Point Click Care, Sigma Care etc.; MDS 3.0 Competent with electronic menu systems: CBORD, Net Menu, Geri Menu etc

Education: Bachelor’s degree in Dietetics, Nutrition, Foods and Nutrition, or a related field from an accredited college or university.

Additional Qualifications

Certifications: Must hold a current Registered Dietitian (RD) credential through the Commission on Dietetic Registration (CDR), subject to Primary Source Verification.
