Responsibilities: Provide personal care assistance to patients, including bathing, dressing, and grooming Assist with daily living activities, such as toileting, incontinence care, and mobility assistance Administer medications as prescribed by a physician Develop and implement personalized care plans to meet individual patient needs Collaborate with healthcare teams to ensure seamless transitions and comprehensive care Maintain accurate and detailed records of patient care and progress
Requirements: 1 years of experience or equivalent training in home health care High school diploma or equivalent required (n/a) Ability to work independently and as part of a team Strong communication and interpersonal skills Compassionate and empathetic approach to patient care
