Position Summary: Village Home Care is seeking a dependable and compassionate Licensed Practical Nurse (LPN) to join our growing home health team. The LPN will provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse and physician.
Key Responsibilities: * Provide direct patient care in accordance with physician orders and care plans* Monitor and document patient condition, vital signs, and progress* Administer medications and treatments as ordered* Educate patients and caregivers on disease management and home care techniques* Assist with wound care, catheter care, diabetic care, and other skilled nursing services* Communicate changes in patient condition to supervising RN and physician* Complete timely and accurate clinical documentation* Participate in interdisciplinary team meetings and care coordination Qualifications* Current Florida Licensed Practical Nurse (LPN) license (required)* Minimum of 1 year nursing experience (home health experience preferred)* Strong clinical, organizational, and communication skills* Ability to work independently in a home setting* Reliable transportation and valid driver’s license* Current CPR certification Why Join Village Home Care?
