Our team is dedicated to providing high-quality, person-centered care and support to individuals with developmental disabilities, and we are seeking a compassionate and motivated individual to join our team.
Responsibilities: Provide support and assistance to individuals with developmental disabilities in their homes, communities, and day programs Develop and implement personalized care plans to meet the unique needs and goals of each individual Assist with daily living activities, such as personal care, meal preparation, and household management Foster positive relationships and provide emotional support to individuals and their families Collaborate with other healthcare professionals to ensure comprehensive care and support Maintain accurate records and documentation of services and progress
