About the role

Easterseals PORT Health is seeking a licensed or license-eligible clinician to join the VA REACH team in Northern Virginia as a Referral and Entry Coordinator. The role focuses on evaluating eligibility, processing referrals, maintaining records, and sharing information with partners. You will also support on-call mobile crisis activities, with a primarily remote arrangement and occasional client home visits.
