We are seeking a dedicatedRN Case Managerto join our collaborative healthcare team in providing exceptional, patient-centered care for an elderly population. In this rewarding role, you'll partner closely with Primary Care Providers and an interdisciplinary care team to coordinate comprehensive care, perform clinical assessments, develop individualized care plans, and support patients throughout every stage of their healthcare journey. What You'll Do: Patient Care & Care CoordinationManage an assigned panel of patients while taking ownership of clinical outcomes and quality of care. Perform comprehensive nursing assessments in the clinic, virtually, and during home visits. Develop, implement, and update individualized Plans of Care in collaboration with providers, patients, families, and caregivers. Educate patients and families on managing chronic illnesses, medications, disease prevention, and healthy living. Identify changes in patient condition and coordinate timely interventions with the healthcare team. Case ManagementCoordinate care across hospitals, emergency departments, skilled nursing facilities, home health, hospice, and specialty providers. Facilitate smooth transitions of care to reduce avoidable hospitalizations and improve patient outcomes. Coordinate referrals for specialty care, laboratory services, procedures, personal care, transportation, vision, dental, and community resources. Monitor hospice transitions and collaborate with hospice agencies when appropriate. Clinical SupportPerform nursing procedures and direct patient care in both clinic and home settings. Manage provider inbox tasks, medical orders, laboratory results, and follow-up care. Communicate changes in patient condition to providers and interdisciplinary team members. Maintain accurate, timely, and compliant electronic health record (EHR) documentation.
What We're Looking For: Required QualificationsActive, unrestricted California Registered Nurse (RN) license. Graduate of an accredited School of Nursing. Current Basic Life Support (BLS) certification. Minimum of 2 years of RN experience caring for frail or elderly adults. Strong clinical assessment and critical thinking skills. Excellent communication, organizational, and multitasking abilities. Ability to work independently while collaborating with an interdisciplinary healthcare team. Comfortable traveling locally for home visits (2–5 times per week). Preferred QualificationsExperience in: Case Management Home Health Hospice Skilled Nursing PACE Programs Geriatric Care Transitional Care Care Coordination Experience with Electronic Health Records (EHR).
