About the role

The Outreach Specialist is responsible for enrollment of prospective participants into the PACE program. Responsibilities include directing and coordinating all marketing, outreach, and community engagement activities, managing intake/enrollment processes, and reporting on team performance with a focus on efficiency and conversion. Collaboration with referral sources and DHCS is required to maintain compliant enrollment workflows.
