About the role

Eddy Visiting Nursing and Rehabilitation Association is seeking a Weekend Track Homecare Physical Therapist to provide patient-centered therapy in patients' homes on Saturdays and Sundays, with 12-hour day shifts. This role offers a $5,000 sign-on bonus and strong clinical support. Responsibilities include admission of home-based patients, delivery of PT services, and contributing to seamless transitions from inpatient to community care. NYS PT license and valid driver’s license required.
