Job Title Physical Therapist – Home Health LocationOwings Mills, MD | Home Health Schedule / Job Type Contract | Full-Time | Monday–Friday, 8:00 a.m. – 4:30 p.m. | Onsite / Field-BasedKey Responsibilities Conduct home visits throughout the assigned service area (primary coverage in Carroll County with support for Baltimore County), with an expected volume of approximately 6 patient visits per day Complete assessments, evaluations, reassessments, plans of care, and discharge documentation, including OASIS documentation Provide gait training, mobility assistance, transfer training, assistive device instruction, and home exercise program development Deliver orthopedic and neurologic rehabilitation services to an adult and geriatric patient population Provide patient and family education focused on safety and injury prevention Document clinical notes and maintain records using the Kinnser/WellSky EMR platform, meeting a productivity expectation of 30 points per week
Required Qualifications:
- Active Maryland Physical Therapist license required upon start; candidates may submit with license pendingBLS certification required
- Minimum of 2 years of Physical Therapist experience required
- Home Health Physical Therapy experience required; first-time travelers are not acceptedOASIS documentation experience requiredEMR experience required; experience with Kinnser/WellSky preferred
- Must be comfortable traveling throughout the assigned service area to complete home visits
- Preferred Experience / Skills
- Experience working with adult and geriatric patient populations in a home health setting
- Background in both orthopedic and neurologic rehabilitation
- Familiarity with Kinnser/WellSky EMR platform
