About the role
- Model, practice, and promote a recovery-oriented approach grounded in trauma-informed care, hope, respect, empowerment, wellness, and connectedness
- Support consumers in developing personal responsibility, self-advocacy, and confidence through professional, compassionate interactions
- Provide consumer-driven services identified in treatment, recovery, and safety plans while maintaining at least 65% productivity of available time
- Teach and reinforce recovery tools, including Wellness Recovery Action Planning (WRAP), and utilize recovery-focused language in all documentation
- Assist consumers with referrals, linkage, and advocacy to community resources, natural supports, internal programs, and discharge placements that promote independence
- Promote positive relationships within Chestnut and with community partners, including substance use providers, hospitals, medical staff, and Medicaid partners
- Complete timely and accurate documentation in accordance with program and funder requirements, including:
- Progress notes and required data entry (80% within 48 hours of service delivery)
- Medication Administration Records completed in real time
- Thorough and timely shift reports
- Participate in meetings, consumer staffings, program development activities, and required supervision and trainings
- Answer incoming calls, manage messages and mail, and monitor video surveillance equipment when applicable
- Collaborate with coworkers, supervisors, facility staff, and tenants to maintain a safe, clean, and supportive living environment
- Maintain confidentiality and protect all consumer and organizational information
