Required clinical activity in the last 24 months (case logs) Applicant must provide documentation (case list) of the provision of clinical services (100 patients) representative of the privileges being requested during the previous 24 months. Case list must include diagnosis or procedure and date, and facility where performed. Will provider work solo?
