About the role

Ellis Medicine is seeking a Per Diem Hospital Medicine Advanced Practitioner to join our collaborative Hospital Medicine team. This is an excellent opportunity for an experienced APP or new graduate seeking flexible clinical work within a well-supported Hospital Medicine program. Our APPs play an integral role in the care of hospitalized patients, working collaboratively with physicians while managing a dedicated patient census. With strong physician support, broad specialty availability, a closed ICU and no required procedures, our APPs can focus on providing high-quality, patient-centered care.

Clinical Responsibilities
Per diem Hospital Medicine APPs will:
Provide comprehensive inpatient care in collaboration with Hospital Medicine physicians
Evaluate and manage hospitalized patients
Perform daily rounds and develop individualized treatment plans
Complete admission and discharge evaluations as assigned
Coordinate care with physicians, nursing staff and consulting specialists
Communicate treatment plans and clinical updates with patients and families
Participate in multidisciplinary care planning
Coordinate transitions of care and follow-up needs
Provide evidence-based, patient-centered care

Qualifications:

Current NP or PA certification
Licensed (or eligible) to practice in New York State
Hospital medicine or inpatient experience preferred but not required
Strong clinical judgment and communication skills
Ability to work collaboratively within a multidisciplinary team
Commitment to high-quality, patient-centered care
