About the role

Family Home Health and Hospice -Family Home Health is a dynamic post-acute care service provider of intermittent home health care, palliative care, hospice care, skilled private care services and custodial private care services. We are looking for a compassionate and experienced Medical Social Worker to join our interdisciplinary team. As a Medical Social Worker, you will assist patients and families to cope with psychosocial challenges to achieving their health care goals. You will utilize a holistic approach to assess patient and family/caregivers needs relative to coordinating appropriate care. You will develop, implement, and update interdisciplinary treatment plans, consult with and educate patient and family and facilitate referrals to other healthcare professionals and programs. You will participate in team meetings and required in-service trainings. The ideal candidate will have: Minimum of 1 year of Medical Social Work experience and possess a MSWPresent a compassionate and caring demeanor with the ability to build a rapport with clients Possess excellent communication and customer service skills, be well organized, and be comfortable with electronic documentationHCHB EMR experience a plus Prior Hospice care experience a plusTHIS POSITION IS 3 DAYS A WEEK. DAYS CAN BE FLEXIBLE. PRIMARILY COVERS EVERYTHING SOUTH OF ADDISON, IL.
