What You’ll Do:
Nutrition assessments and patient care planning.
Monitor weight changes, nutritional status, wounds, and nutrition-related concerns.
Provide recommendations for therapeutic diets, supplements, and tube feeding when appropriate.
Monitor weight changes, nutritional status, wounds, and nutrition-related concerns.
Educate patients regarding nutrition to promote wellness.
Collaborate with Physicians, Nurses, Speech & Occupational Therapists, and the interdisciplinary care team.
Help improve patient outcomes using “Food as Medicine”.
