Position Summary:
Evaluates patients' social, emotional, environmental, financial, and support needs to identify barriers to care and promote positive health outcomes. Provides psychosocial assessment, crisis intervention, care coordination, and advocacy services while collaborating with patients, families, and the interdisciplinary care team to support treatment goals and discharge planning.
Essential Duties & Responsibilities:
Conduct comprehensive psychosocial assessments to identify patient and family needs, risks, and barriers to care. Develop and implement intervention plans that support patients' health, well-being, and access to community resources. Provide counseling, emotional support, and crisis intervention for patients and families facing complex situations. Collaborate with care managers, physicians, nurses, and community partners to coordinate care and discharge planning. Facilitate patient and family decision-making by providing education, guidance, and advocacy. Connect patients with financial assistance programs, social services, and community-based resources. Maintain accurate, timely, and confidential documentation in accordance with organizational and regulatory requirements. Promote compliance with healthcare regulations, ethical standards, and social work best practices.
Job Requirements:
Licensed Social Worker (LSW) in Tennessee required. Bachelor's degree in social work (BSW) required. One (1) year of social services experience in a hospital or healthcare setting required.
