About the role

MedZed seeks a Bilingual Community Health Navigator II for Fremont/Newark. This full-time, field-based role combines in-the-field visits with remote work to enroll and support Medicaid members in the Oakland area. The CHN II will manage complex cases, maintain care plans, and ensure documentation and CalAIM compliance while traveling to meet members where they live. Hybrid/remote location supports field-based outreach and engagement.
