Location: Nassau County (with occasional cases in Queens, Manhattan, and Brooklyn, NY)

Position Summary: We are seeking an outstanding Per Diem Registered Nurse (RN) to join our growing team. This role offers flexibility, a very competitive compensation, and an opportunity to work in a supportive, professional environment. Most assignments are in Nassau County and Queens, with some casses in Manhattan, and Brooklyn. The RN will provide clinical supervision of home health aides, conduct patient in-home assessments, develop care plans, and collaborate with our team to ensure continuity of care.

Key Responsibilities: Conduct in-home patient assessments and develop individualized care plans. Supervise, support, and train home health aides to maintain quality of care. Communicate effectively with patients, families, and interdisciplinary team members. Document clinical findings and maintain compliance with agency and state regulations. Work both independently and collaboratively within a team setting. Availability to work some weekends as needed.
Qualifications: Active New York State RN license (required)Strong communication and interpersonal skills Ability to work both independently and as part of a team Excellent organizational and clinical assessment skills Reliable transportation to travel to patient homes in Nassau County, Queens, Manhattan, and Brooklyn
