Requirements:
MD/DO from an accredited medical school
Completion of an Ophthalmology residency program
American Board Eligible or American Board Certified in Ophthalmology
Ability to obtain an unrestricted Pennsylvania Medical License and ability to obtain necessary credentials
DEA certification
