About the role
Grace at Home is seeking an Advanced Practice Practitioner (NP/PA) to conduct in-home and telehealth visits, manage complex chronic conditions, and participate in value-based care initiatives. In this exempt, full-time role, you will collaborate with a clinical team to deliver high-quality care to patients in their homes and communities.
Requirements include a Master’s degree, active NP/PA license, and 2+ years of direct patient care.
