Certification & Licensure:

Board certified or Board eligibility in Family Medicine or Internal Medicine.
Valid medical license (or eligibility) in CT.DEA registration and state-controlled substance certification, or eligibility to obtain both.
Maintained credentialing in accordance with medical staff bylaws and regulatory requirements.
Current BLS certification
